• Be sure your passport has at least 3 months remaining on it from your last day of tour(email us if not at info@zagsports.com and we can double check options)

  Vaccine Declaration Form

  • Currently(this may change), each traveler is required to fill out a vaccine declaration form to enter the Netherlands. Print it, fill it out and bring it with you along with your hard copy of your vaccination card. If you have any questions about COVID vaccination or restrictions email us at info@zagsports.com.

• Permission for a Minor to Travel Form

  • This form is required for any travelers who are not traveling with BOTH parents or only traveling with 1 parent. If only one parent is traveling, the other parent must give permission for the other parent to leave the country. If you’re not traveling with either parent, you both need to give permission for your daughter to travel internationally. You will need to fill out and notarize the “Permission to Travel” form (most banks will notarize for free if you’re a client). For those traveling without both parents, please put your coaches name as the “Accompanying Adult”.
